The Sensor Said Pass. The Nut Was Still Loose.

THE SENSOR SAID PASS. THE NUT WAS STILL LOOSE.

Sometimes the leader does not need to solve the blocker. The leader needs to recognize whose hands it belongs in.

The inspection station says pass.

Production keeps moving.

Then somebody finds a condition that should not have passed.

Now the shift has a problem.

But it may not be the problem the supervisor is qualified to solve.

That distinction matters.

General Motors is recalling 26,541 model-year 2025 and 2026 Chevrolet Express and GMC Savana vehicles because certain steering gear assemblies may contain a nut that was not properly tightened. NHTSA reports that a malfunctioning part sensor at supplier Nexteer's facility allowed some assemblies to pass inspection without the nut being properly tightened. In some cases, the nuts were only hand-tightened.  

A loose nut that separates while the vehicle is being driven can cause loss of steering control and increase crash risk. GM's owner-notification mailing is scheduled to begin August 10, 2026.  

The technical details matter.

But the leadership lesson is simpler.

When the normal process has already produced the wrong signal, the answer is not automatically more effort from the same people using the same capability.

Sometimes the blocker needs different hands.

That is where Manual Engagement matters.


The Leadership Trap

A supervisor sees the problem.

The line is waiting.

People are looking for direction.

Production wants an answer.

Quality wants confidence.

The shipment clock has not stopped.

That creates a strong instinct:

I am responsible, so I need to solve this.

That sounds like ownership.

It can become overreach.

A production supervisor may understand the operation extremely well and still not have the controls knowledge, quality authority, engineering judgment, system access, or trace-data access needed to determine why an automated inspection accepted a bad condition.

Responsibility for the next move does not automatically create expertise over every technical question inside that move.

That is where developing leaders can get pulled too far into the problem.

They start troubleshooting equipment they do not own.

They call everyone into the same discussion.

They ask operators to inspect harder.

They start assigning checks without understanding what the failed signal means.

Or they keep escalating through general channels while the one person who can actually interpret the blocker is still outside the conversation.

Leadership does not mean becoming the specialist every time a specialist is needed.

It means recognizing when the problem has crossed your capability boundary and getting the correct human capability onto that point.


What Usually Happens Under Pressure

Imagine an assembly cell where an automated check normally confirms a fastening condition.

The station reports pass.

Then a physical condition is discovered that does not agree with that signal.

The supervisor has an immediate problem.

Can the signal still be trusted?

What else may have passed under the same condition?

Is the issue with the part, the fastening process, the sensor, the inspection logic, the trace record, or something else?

Those are legitimate questions.

They are also technical questions.

The pressure makes it tempting to do one of three things.

Keep production moving because the system says the part passed.

Stop everything and personally start diagnosing the system.

Or tell everybody on the shift to manually check more things until someone feels confident again.

All three responses can look responsible.

None automatically produces the technical judgment the blocker requires.

This is the point where more activity can become a substitute for the missing capability.


Field Note

Owning the problem does not mean personally possessing every capability required to clear it.

A leader close to the work should be able to identify the mismatch.

The leader should be able to explain what objective is now exposed.

The leader should know when the normal process is no longer giving enough confidence.

But when the blocker requires specialized technical knowledge, quality authority, system access, equipment history, or engineering judgment, the leadership move changes.

The question becomes:

Who has the capability this blocker now requires?

That is a Manual Engagement question.  


Real-World Scenario: General Motors and a Failed Inspection Signal

The verified GM case begins with a dealer report.

On May 28, 2026, a GM brand quality manager submitted a report through GM's Speak Up For Safety program after a dealer reported a loose nut in the steering gear assembly of a 2025 Chevrolet Express. GM opened a product investigation on June 2.  

GM's supplier identified a malfunctioning part sensor that had allowed some steering gear assemblies with only hand-tightened nuts to pass inspection. The supplier then reviewed torque trace data to identify suspect steering gear assemblies. GM's investigation identified nine potentially related field issues beyond the original vehicle, with no reported crashes or injuries among those issues. GM's Safety Field Action Decision Authority decided on June 18 to conduct a safety recall.  

The supplier contained the manufacturing condition on May 29, according to the recall report. GM states that vehicles produced after June 1 were built with steering gear assemblies containing properly torqued nuts.  

That is what the public record establishes.

It does not tell us which supervisor first saw the manufacturing condition.

It does not tell us the internal conversations at Nexteer.

It does not tell us exactly who was assigned to troubleshoot the sensor.

It does not tell us what instructions were issued at each workstation.

We should not invent those details.

But the verified sequence gives us the operating pattern:

A normal inspection signal had already become unreliable.

That matters because once the inspection system itself is part of the uncertainty, simply asking the shift to continue trusting the same signal does not resolve the blocker.

The investigation required technical identification of the sensor issue and access to torque trace data to understand the potentially affected population.  

That is the human-capability point.


Translate That Down to the Production Floor

Now move the same leadership problem to the level of a production supervisor.

This is an illustrative scenario based on the verified operating condition. It is not a claim about what happened inside GM or Nexteer.

You are supervising an assembly area.

The schedule is live.

The customer shipment matters.

Your operators are following the approved work.

The inspection station has been producing pass signals.

Then one assembly is physically checked and the fastening condition does not match what the inspection result suggested.

Now your shift is waiting on you.

Your responsibility is real.

But your authority and technical competence have boundaries.

You know enough to recognize the contradiction.

You do not automatically know why the contradiction exists.

Maybe it is the sensor.

Maybe it is the process.

Maybe it is the data.

Maybe it is a component condition.

Maybe something else has changed.

You do not have enough evidence to responsibly choose among those explanations.

The weak leadership response is:

I am the supervisor. I need to personally figure this out.

That can quickly turn a leadership responsibility into unsupported technical decision-making.

The other weak response is:

Everybody check everything until we feel better.

Now labor is moving, production is waiting, and nobody has improved the technical read.

The better recognition is narrower.

This blocker requires a capability my current path does not have.

That may mean the quality owner.

It may mean the controls specialist.

It may mean the maintenance lead.

It may mean the manufacturing engineer.

It may mean another technical owner with the system history and access necessary to interpret what failed.

The supervisor's job is not to magically become that person.

The supervisor's job is to recognize that the person is needed.

That is Manual Engagement.


The Problem Path

The normal operating path might look straightforward:

Assembly is completed.

The required inspection occurs.

The inspection result supports the next production decision.

Work moves forward.

Then one piece of that path becomes unreliable.

The inspection says pass.

The physical condition says something does not line up.

The immediate temptation is to keep treating this like a normal production problem.

It is not.

The path has reached a blocker that now requires a different human capability.


The Blockage

The blockage is not simply the loose nut.

That is the physical condition that revealed the problem.

The leadership blockage is the loss of confidence in what the inspection signal means.

The production supervisor can recognize that problem.

The operator can report what they see.

But neither should be forced to invent the technical explanation because leadership wants the schedule back.

That is where leadership has to separate:

Who discovered the blocker

from

Who has the capability to interpret and clear the blocker.

Those may be different people.


The Decision Point

This is where many leaders overreach.

They think stepping in means personally taking over.

They start directing technical troubleshooting.

They begin interpreting data outside their expertise.

They expand the issue into a large meeting.

They involve five people who each know part of the problem but nobody who owns the actual point of control.

That is not stronger leadership.

It is more hands on the problem without necessarily putting the right hands on the blocker.

Manual Engagement changes the question.

Not:

How much leadership attention can I put on this?

But:

What capability does this point actually require?

That distinction keeps the leader responsible without pretending responsibility makes them the subject-matter expert.


The Next Movement

Once the blocker is in the right hands, the larger operation still needs leadership.

Production status still needs clarity.

The affected work still needs control under the organization's approved procedures.

Quality still needs a reliable basis for the next decision.

Planning may need updated information.

The team still needs to know what it owns and what it does not own.

The person handling the technical blocker should not become the permanent owner of the entire production problem.

That is another important boundary.

Manual Engagement is targeted.

The right person handles the point that requires their capability.

Then the work returns to the appropriate operating path.

The public blog stops there.

Consequence Chain

When the wrong capability stays on the blocker, the cost begins to spread.

A questionable inspection result creates uncertainty.

Uncertainty creates hesitation or unsupported action.

Unsupported action can create additional suspect work.

Suspect work creates containment, inspection, rework, and schedule pressure.

Schedule pressure starts influencing technical decisions.

Now the team is trying to recover output while confidence in the process is still weak.

That is how a technical blocker becomes a leadership problem.

The opposite mistake creates another chain.

The supervisor personally takes over.

The team waits for the supervisor.

Every difficult technical issue starts flowing upward.

The supervisor becomes the permanent point of intervention.

Technical owners enter late.

The team learns that difficult problems belong to the manager instead of the appropriate capability owner.

Now Manual Engagement has been replaced by dependency.

The leader became the blocker while trying to remove one.


The Better Read

The better read is not:

There is a technical problem. I need to solve it myself.

The better read is:

There is a technical blocker. I need to determine what capability is required before I decide who should handle it.

That is a realistic decision for a supervisor, shift leader, team lead, project lead, or workstream owner.

You do not need executive authority.

You do not need to run the engineering organization.

You need enough judgment to recognize when the current team can continue through normal process and when the blocker requires a person with different authority, access, technical knowledge, or decision rights.

That is a leadership skill.


How This Fits the Direct Action System

Manual Engagement sits inside Decision Execution and Problem Navigation.

Manual Engagement becomes relevant when the limiting point is not simply process, routing, delay, or stabilization.

From there, risk still matters.

The wrong person, too much authority, or an engagement that expands beyond the actual blocker can create a second problem.

The broader system supports the decision.

But Manual Engagement remains the point here:

Put the right human capability on the blocker without turning that engagement into a takeover.  


The Point

GM's recall gives us a useful real-world signal.

A manufacturing inspection process indicated assemblies had passed.

A malfunctioning sensor had allowed some assemblies with improperly tightened nuts to pass that inspection.

The response required more than another routine pass through the same process. Technical investigation and torque trace data were used to identify what had happened and which assemblies were suspect.  

For the leader close to the work, the lesson is not how to troubleshoot a steering-gear sensor.

The lesson is simpler.

Know when the problem has reached the edge of your capability and needs the right person, not more general effort.

That is not giving away responsibility.

That is applying responsibility correctly.


A Practical Field Exercise

Use this the next time a production issue reaches a point your normal process cannot cleanly resolve.

This is a recognition exercise, not the full Manual Engagement method.

1. Name the Contradiction

What does the normal process say?

What does the physical condition, quality signal, machine behavior, or operating result say?

Do they agree?

If they do not, stop treating the normal signal as though nothing changed.

2. Identify the Missing Capability

Ask:

Does this require technical expertise?

Quality authority?

Equipment history?

System access?

Engineering judgment?

A decision right I do not hold?

You are not selecting the full response yet.

You are identifying what the current path is missing.

3. Separate Ownership From Expertise

Ask:

What part of this still belongs to me?

What part requires somebody else's capability?

A supervisor can own the production decision without pretending to own the engineering determination.

4. Watch the Scope

Once the specialist enters, do not automatically hand them the entire operation.

The blocker needs their capability.

The larger objective still needs normal leadership, communication, and ownership.


What Leaders Should Watch For

The Same Problem Keeps Returning to a General Meeting

If ten people keep discussing the issue but the blocker does not move, ask whether the required capability is actually in the room.

The Supervisor Has Started Doing Specialist Work

Stepping up is useful.

Operating outside your competence or authority because nobody else has been engaged is not.

Everyone Is Checking More, but Nobody Knows What the Failed Signal Means

More inspection activity does not automatically create better information.

The Team Is Waiting on Authority Nobody Present Actually Has

Coordination cannot manufacture decision rights.

One Expert Has Become the Permanent Fix for Every Problem

That is no longer targeted engagement.

That is dependency, and it is exposing a deeper process or capability problem.

Nobody Knows When the Specialist's Role Ends

If the technical owner enters the issue and never hands the work back, a temporary engagement can quietly become permanent ownership.


Why This Matters for Manufacturing

Manufacturing puts leaders close to technical work.

That proximity creates a dangerous illusion.

You see the machine every day.

You know the line.

You know the operators.

You understand the schedule.

You have lived through downtime, quality holds, changeovers, material shortages, and customer-critical runs.

That experience matters.

It still does not make every supervisor a controls engineer, quality engineer, maintenance specialist, process engineer, or technical authority.

Manufacturing leadership requires knowing enough about the operation to recognize when a problem has changed categories.

A line-rate problem may belong inside normal supervision.

A disagreement between an automated acceptance signal and the actual physical condition may require another capability entirely.

The leader does not become weaker by recognizing that.

The leader becomes more accurate.


Where Manual Engagement Fits

This is where Manual Engagement fits.

It helps leaders recognize blockers that normal process cannot move because the missing control is human capability.

That capability may be technical skill.

Authority.

Judgment.

Trust.

Relationship ownership.

Direct access.

Manual Engagement is not micromanagement.

It is not the leader doing everybody else's job.

It is not bringing more people into the problem.

It is recognizing which person is necessary and why.

What to Practice This Week

The next time your line, cell, project, or shift hits a blocker, ask:

What exactly is stuck?

Is normal process actually capable of resolving it?

What capability is missing from the current path?

Am I about to take over work that belongs to a specialist or authority owner?

Who has the capability to handle this point without taking over the entire problem?

Do not turn those questions into another meeting.

Use them to recognize when the problem needs different hands.


Final Thought

The GM case started with a physical condition that did not agree with what the inspection process had allowed through.

That disagreement mattered.

GM's investigation ultimately identified a malfunctioning supplier sensor, used torque trace data to identify suspect assemblies, contained the manufacturing condition, and moved into a safety recall.  

The leadership lesson sits below that corporate response.

A supervisor does not need to personally diagnose every technical blocker.

A team lead does not need executive authority to recognize that the wrong capability is being applied.

A leader's responsibility is not to become every expert in the building.

It is to recognize when the blocker requires the right expert, authority, judgment, trust, or access, and get that capability onto the point that is actually stuck.

Put the right hands on the blocker.

Do not put every hand on the problem.
